How do you keep split washers from spreading too far?

During the torque of the bolt the washer is spreading too far. We cant lower the torque range and the spread of the washer must stay under 4mm. I really need a good answer. Someone that has had this issue before. How do you keep split washers from spreading too far?
Generally, when the washer spreads beyond the underside of the bolt head, the split washer is not the proper size. It should ideally fit outside of the bolt head's camfer so it isn't squeezed outward but rather simply squashed under the flat portion. How do you keep split washers from spreading too far?
Huh? Unless it's a really unusual application, the washer shouldn't spread AT ALL. Obviously if it's spreading out from under you, you don't have a chance of holding the torque you need.

It sounds to me like that washer is broken/cracked and needs to be replaced. Maybe there's something wrong with the bolt head or surface, but that doesn't seem likely to me.

Maybe I'm misunderstanding your application. Can you find a picture on the web of a washer similar to this one?
use the correct size. Normally on high torque bolts split washers are not used.
you need new bolts and or washers at least washers
